Referrals

A primary care provider must refer patients on managed care plans. Self referrals are acceptable in certain circumstances. When making a physician referral, please complete a UW Medicine referral request.

Referrals may be made to the following UW Medicine clinics:

• Neurological Surgery Clinic at Harborview Medical Center

• Neurological Surgery Clinic at UW Medical Center

• Alvord Brain Tumor Clinic at UW Medical Center

• Cerebrovascular Lab at Harborview

• UW Medicine Gamma Knife Center at Harborview Medical Center

• Seattle Sports Concussion Program at Harborview

• EpicCare Link Provides On-line Access to Medical Records

All physicians referring to neurological surgery will be offered the option of obtaining a secured password and directions for accessing your patient's medical records online using EpicCare Link.
